·放射与影像· 2019年11月第9卷第21期乳腺良性肿块微创手术后术野超声图像变化的研究史文婷 张相机 卢 璐 林 慧广东省湛江市妇幼保健院乳腺外科,广东湛江 524038[摘要] 目的 分析乳腺良性肿块微创手术后术野不同时期超声图像变化的特点。 方法 收集2016年6月~2019年1月在湛江市妇幼保健院乳腺外科行乳腺良性肿块麦默通微创手术术后随访病例,共402例,将原发病灶按最大径线<1cm(A组),1~2cm(B组),>2cm(C组)分成三组各134例。于术后1周、1、3、6个月行手术部位超声检查和X线钼靶检查,再对不同时期超声图像变化及X线钼靶图像变化的特点分析统计,对比两种检查方式在乳腺良性肿块微创手术后的应用价值。 结果 良性乳腺肿块的超声显示纵横比多<1、形态比较规整,边界清晰,表面光滑,内部回声均匀,肿块后方回声无衰减,后壁回声清晰,无周围组织浸润,可活动,移动性好。对所有患者术后1周,3、6个月展开多普勒超声检查,A组伤口疼痛持续时间明显及引流管拔除时间明显短于B组和C组,腋窝和胸壁引流量低于B组和C组,差异有统计学意义(P<0.05);发生血肿有26例,其中C组血肿发生率最高,血肿吸收时间最长,与其他组别相比差异有统计学意义(P<0.05),而X线仅检测出18例术后血肿,与超声检出率对比差异有统计学意义(P<0.05)。 结论 通过超声检查乳腺良性肿块麦默通微创手术患者,有助于总结其声像学特点并进行区分,为该类病例的处理提供依据,避免不必要的二次手术。[关键词] 乳腺良性肿块;微创手术;超声;病理性质[中图分类号] R737.9 [文献标识码] A [文章编号] 2095-0616(2019)21-174-04Study on the changes of ultrasound image in the surgical field after minimally invasive surgery for benign breast massesSHI Wenting ZHANG Xiangji LU Lu LIN HuiBreast Surgery,Zhanjiang Maternal and Child Health Hospital,Guangdong,Zhanjiang 524038,China[Abstract] Objective To analyze the characteristics of ultrasound image changes in different surgical field periods after minimally invasive surgery of breast benign masses. Methods 402 follow-up cases of mammary benign mass after minimally invasive operation were collected from breast surgery of Zhanjiang Maternal and Child Health Hospital from June 2016 to January 2019.The primary lesions were divided into three groups,with 134 cases in each group according to the maximum diameter <1cm(group A,1-2cm(group Band >2cm(Group C.One week,one month,three months and six months after the operation,ultrasound examination and X-ray molybdenum target examination were performed.Then the characteristics of ultrasound image changes and X-ray molybdenum target image changes in different periods were statistically analyzed,and the application value of the two examination methods in the minimally invasive operation of breast benign masses was compared. Results The ultrasound of benign breast mass showed that the aspect ratio was less than 1,the shape was relatively regular,the boundary was clear,the surface was smooth,the internal echo was uniform,the echo behind the mass had no attenuation,the echo of the back wall was clear,and there was no infiltration of surrounding tissue,so it was movable and had good mobility.All patients were examined by Doppler ultrasound 1 week,3 months and 6 months after operation.The duration of pain and drainage tube removal time in group A were significantly shorter than those in group B and C,and the volume of axillary and chest wall drainage were significantly lower than those in group B and C(P<0.05.There were 26 cases of hematoma,among them,group C had the highest incidence of hematoma and the longest absorption time of hematoma,the difference was statistically significant compared with other groups(P<0.05.However,there were only 18 cases of postoperative hematoma detected by X-ray,which was significantly different from the detection rate of ultrasound(P<0.05.
